Is it posible to output intremental coordinates with Mill CAM Designer?
Also how can I set a profile for a machine whos 0,0 is at the top right of the bed?

Which CNC controller are you using?
MCD can create incremental output, but it is a real pain to configure as it requires a new post processor file (SLB file) creating to accomodate your controller.
Setting the zero co-ord to the top-left is also possible, but the same pain again I'm afraid
